    Distance Education in Higher Education: The Perspective of Key Social Actors

    This article considers the perspective of the key social actors (students, teachers and university management) regarding distance education development in Russia. In the modern information and digital age, distance education – based on the possibility of disseminating knowledge through modern computer technologies – is being actively introduced into the Russian higher education system. The pace of implementation is increasing in Russia, yet its assessment as a new form of learning and teaching is ambiguous. There are clear advantages to this form when compared with traditional face-to-face classroom instruction including: the accessibility of education to a wide audience; the possibility to earn a degree irrespective of place of residence; variability of learning time selection; relatively low tuition fees. At the same time, however, many believe that the quality of education received by distance students is lower than that received via face-to-face teaching, due to both the lack of student’ readiness and the difficulties related to pedagogical process organization and management. This study considers whether distance higher education will continue to develop in Russia. The answer to this question depends to a large extent on the perspective of the key social actors. The study analyses their attitudes to distance education and make the following conclusions: 1) University management generally accept this form because of its economic advantage and attitudes of the higher education authorities. Students also vote in favour, because it allows them to earn a degree without giving up work, family and home-related commitments. The most negative assessment is given by teachers who conduct classes at a distance and can compare distance learning outcomes with those in the traditional face-to-face classroom instruction.     Educational Reforms in the Context of Contemporary Civilizational Changes

    Analysis of educational system seems to be the most reasonable when the key points are reformation and modernization of education, since the mentioned processes are the response to transformations in adjacent social spheres caused in their turn by objective changes, which may include civilizational, transnational changes in education at national level. This article is devoted to the estimation of educational reforms in the context of contemporary civilizational changes. The research aimed to systematize the features of development of reforms in the field of education, to develop measures on coordination of activities among main participants in educational process, to determine its main features in the context of civilizational changes. It has been established that the key point is to detect balance between autonomous right of a university to render education and scientific studies for economy and society, and responsibility of state for assurance of quality, accountability, integrity, and availability of education. It has been proved that the education sector needs investment to improve learning and research conditions. Another issue is implementation of new forms of financing, including on competitive basis, to support interest and interaction between universities and business environment. It has been determined that new trends in cooperation should not only concern the development of innovations and commercialization but also attract employers to cooperation aiming at development of new or updating the existing learning programs

    According to key diabetic studies, the early use of metformin glucose lowering therapy is associated with a reduced risk of developing micro- and, in the long term, 10-year follow-up, macrovascular complications and cardiovascular mortality. Short-term studies results on combined glucose lowering therapy with metformin suggests that combination therapy can have several advantages on the one side from the effectiveness of glycemic control and on another side from positive effect on the development of complications of type 2 diabetes. The question of the start time of combined hypoglycemic therapy remains open. According to the results of recent large-scale studies, real world evidence data, careful glycemic control during the first year from the moment of diagnosis of type 2 diabetes is crucial for further management of the disease and slow the progression of complications. However, due to the fact that the clinical benefits of early combination therapy were not demonstrated in randomized clinical trials, this approach, despite the theoretical background, was not recommended for widespread use in international guidelines for the treatment diabetes patients. Russian algorithms on the treatment diabetes patients recommend combined glucose lowering therapy at the start of treatment at a HbA1c level of 1% higher than the target. A 5-year VERIFY study results were demonstrated long-term sustained glycemic control in combination with vildagliptin + metformin prescribed for native diabetes patients with relatively low HbA1c values, as well as the advantages of this approach in comparison with the standard strategy for phased intensification of monotherapy. The results of the VERIFY study provided a wealth of information to discuss early treatment intensification, the clinical benefits of this approach and a possible review of the treatment strategy for native diabetes patients

    Effects of antiplatelet therapy on stroke risk by brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases: subgroup analyses of the RESTART randomised, open-label trial

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage.     Background: Antiplatelet therapy reduces the risk of major vascular events for people with occlusive vascular disease, although it might increase the risk of intracranial haemorrhage. Patients surviving the commonest subtype of intracranial haemorrhage, intracerebral haemorrhage, are at risk of both haemorrhagic and occlusive vascular events, but whether antiplatelet therapy can be used safely is unclear. We aimed to estimate the relative and absolute effects of antiplatelet therapy on recurrent intracerebral haemorrhage and whether this risk might exceed any reduction of occlusive vascular events. Methods: The REstart or STop Antithrombotics Randomised Trial (RESTART) was a prospective, randomised, open-label, blinded endpoint, parallel-group trial at 122 hospitals in the UK. We recruited adults (≥18 years) who were taking antithrombotic (antiplatelet or anticoagulant) therapy for the prevention of occlusive vascular disease when they developed intracerebral haemorrhage, discontinued antithrombotic therapy, and survived for 24 h. Computerised randomisation incorporating minimisation allocated participants (1:1) to start or avoid antiplatelet therapy. We followed participants for the primary outcome (recurrent symptomatic intracerebral haemorrhage) for up to 5 years. We analysed data from all randomised participants using Cox proportional hazards regression, adjusted for minimisation covariates. This trial is registered with ISRCTN (number ISRCTN71907627). Findings: Between May 22, 2013, and May 31, 2018, 537 participants were recruited a median of 76 days (IQR 29–146) after intracerebral haemorrhage onset: 268 were assigned to start and 269 (one withdrew) to avoid antiplatelet therapy. Participants were followed for a median of 2·0 years (IQR [1·0– 3·0]; completeness 99·3%). 12 (4%) of 268 participants allocated to antiplatelet therapy had recurrence of intracerebral haemorrhage compared with 23 (9%) of 268 participants allocated to avoid antiplatelet therapy (adjusted hazard ratio 0·51 [95% CI 0·25–1·03]; p=0·060). 18 (7%) participants allocated to antiplatelet therapy experienced major haemorrhagic events compared with 25 (9%) participants allocated to avoid antiplatelet therapy (0·71 [0·39–1·30]; p=0·27), and 39 [15%] participants allocated to antiplatelet therapy had major occlusive vascular events compared with 38 [14%] allocated to avoid antiplatelet therapy (1·02 [0·65–1·60]; p=0·92). Interpretation: These results exclude all but a very modest increase in the risk of recurrent intracerebral haemorrhage with antiplatelet therapy for patients on antithrombotic therapy for the prevention of occlusive vascular disease when they developed intracerebral haemorrhage. The risk of recurrent intracerebral haemorrhage is probably too small to exceed the established benefits of antiplatelet therapy for secondary prevention

    Educators’ and students’ perceptions of online distance education before and amid COVID-19: Key concerns and challenges

    This paper aims to present Russian educators’ and students’ perceptions of online education before and during the pandemic. The author analyses the primary research data collected by the team of USUE authors via quantitate research methods, and secondary research data. Based on the empirical sociological data the author identifies the key challenges facing educators’ community – negative attitudes to online teaching and learning, lack of digital pedagogy skills and ineffective interaction practices. It is concluded that in the emergency situation teachers overall managed to fit rapid curriculum transformations. Yet, university leadership and educators should come up with solutions how to address the identified challenges in online distance teaching. These solutions can involve conducting in-depth research into digital pedagogy, disseminating knowledge about the specifics of online interactions and running professional development programs for the university teachers to enhance their performance in courses delivered online

    New paradigms and challenges of social life in the information and digital era

    The information-digital era has led to the emergence in our social life of a number of new paradigms - social standards and patterns of behavior, new values and beliefs that are implemented in all spheres of society. The paper aims to analyze the emerging paradigms regarding the changes they introduce in our social lives and threats brought about by these changes. The authors analyze such paradigms as a new attitude to information, the introduction of digital technologies, changes in the qualification level of the workforce, new features of the lifestyle, and a change in spatial behavior. It is concluded that each paradigm introduces behavioral standard modifications to social life and makes social activity easier in terms of physical costs. At the same time, each new paradigm simultaneously generates new challenges for a person: problems of selecting information and assessing its reliability, arising information inequality, underdevelopment of digital culture, a shortage of relevant specialists, and the disappearance of a number of professions. Overall, the authors view these challenges as inevitable consequences of growth that humanity must explore and learn to overcome
